Full Facelift

A full facelift is really designed to improve the lower 1/3 of the face and the neck. A midface lift, in contrast, will elevate the soft tissues of the cheeks. These two procedures can be combined in selected patients who have flattened cheeks as well as jowls and neck sagging.

A Neck Lift is a procedure that focuses on the neck. A Neck Lift usually involves making a small incision under the chin and around the lower part of the ears. The underlying muscle layer is tightened and the extra skin is removed around the ears. Liposuction of the neck may also be used.

A facelift does not improve the appearance of the eyes, but this can be easily accomplished with a procedure called a blepharoplasty, or “eyelid lift”. This procedure involves removing excess tissue of the eyelids to create a younger, more refreshed look. You may also want to consider a brow lift, to raise the eyebrows to a more youthful position.

This crease is called the “nasolabial fold”, and it is usually not improved significantly by a facelift. The nasolabial fold is best treated with fillers, transferring fat or by performing a midface lift which elevates the soft tissue of the cheek.

You can, and often patients decide to have their eyes, brows, and/or midface improved at the same time. Doing multiple procedures on the same day does not lengthen the recovery time, and it allows the patient to accomplish many goals with one surgery.

Your face will always look younger and more refreshed after surgery, but it will continue to age. After a full facelift, some patients may want a revision surgery, usually ten or more years later.

Mini Facelift

However, mini facelifts have shorter recovery times, are less expensive, and still lead to an impressive aesthetic improvement.

A mini facelift concentrates on the area of the lower face only.

Mid Facelift

The best midface lift patient is in good health, a non-smoker, and is able to avoid blood thinning medications for two weeks before surgery. He or she is unhappy with the loss of fullness in the cheek regions, and the presence of deep smile lines.

A midface lift is intended to lift the cheek area or the middle 1/3 of the face. In contrast a facelift improves the lower 1/3 of the face and the neck, from the corners of the mouth to the base of the neck. In many patients these two procedures are performed at the same surgery, for a more complete facial rejuvenation.

Aside from a midface lift, the best treatment options for “smile lines” (or nasolabial folds) is the transfer of fat from other areas of your body or “off the shelf fillers such as Restylane, Juvaderm Radiesse, Sculptra and others. All of these products give an effective result which is temporary and requires maintenance with periodic injections or fat transfer. Remember that filling the cheek or nasolabial folds will not reposition and elevate the cheek tissues like a midface lift.

Injecting various off the shelf fillers are effective, but need to be re-injected to maintain the appearance you desire. Depending on the product and the amount of correction desired you may need to be re-injected every 6- 24 months. There are many several different types and brands of fillers. We will discuss your goals and recommend the best products to achieve your goals.

Cheek implants are effective at replacing lost volume of the cheeks. They are permanent, but can be removed at any time if needed. They do not lift the cheeks, but they do create full cheeks with a similar appearance to that generated by a midface lift. We will explain all options available to reach your desired goals.

Yes, and that is why it is in high demand!  The result is very natural. There are 2 small incisions on each side which are not visible. One is in the mouth and the other one is in the hair. The recovery is quick, generating a young healthy appearance. Immediately after surgery the cheeks may appear unnaturally high but this resolves quickly during the healing process.

Yes. Patients often decide to have other rejuvenating procedures done at the same surgery. These can include a facelift, eyelid lift (blepharoplasty), brow lift, and neck lift. Ears / Otoplasty

Ears that “stick out” too much are called prominent ears.  Otoplasty is the medical word used for procedures that make the ears look better.

Ear pinning is the colloquial term that is used for an otoplasty – though no pins are used in the procedure.

Starting at age 6-8 the ears have reached their ideal size. It is also around the time that teasing begins. People of all ages seek surgery to improve the appearance of the ears. If you are a parent or an adult with any concerns please schedule a consultation.

It is always desirable to have our eyes noticed, but never our ears. The goal of an otoplasty is to make the ears blend into the head and not be noticed. Ears come in an infinite variety of shapes, and in an otoplasty a surgeon has a great ability to sculpt the ears. There are two main parts of the issue that are usually the focus of an otoplasty. The first is whether the concha – the bowl shaped base of the ear – is too large. When large, it can be reduced or rotated back against the head. The other issue is whether the beautiful folds that make up the ear are all properly intact. One such fold – the antihelical fold – is often absent or weak in patients who need an otoplasty. By folding it back and holding it in place with sutures, its shape can be improved and made much more attractive.

Yes! It is very common for an adult to get an otoplasty. Their parents may not have noticed it, perhaps no one knew surgery was an option, or perhaps the parents felt that it didn’t bother the child. Some parents think it is one of the features that make a child cute and that they will outgrow it.

Adult otoplasty patients are among the most enthusiastic plastic surgery patients of all. Women will say that they have always wanted to wear their hair back but never could because of their ears. Many women mention that the tops of their ears protrude through their hair even when their hair is back. Men are even more bothered by prominent ears than women, perhaps because their shorter hair makes them harder to conceal. And there is probably a psychological component as well. As much as a strong jaw is associated with masculinity, wide ears are associated with a childlike, less mature, or a less serious personality.

General anesthesia is the preferred anesthesia for children, but adults can be very comfortable with local anesthesia.

Nose / Rhinoplasty

A number of changes to the shape of the nose can be made with rhinoplasty. Noses can be made smaller or larger, narrower or wider, shorter or longer. Bumps on the bridge can be corrected and droopy noses can be lifted.

The surgery is performed by reshaping the cartilage and bone under the skin of the nose. After these deep structures are reshaped appropriately, the skin re-drapes over the new framework.

Most of the incisions (cuts) for rhinoplasty are made inside the nose. There are situations that require small incisions to be made outside the nose. These are typically placed in well-hidden areas.

Typically, most of the incisions are inside the nose and the small incisions that may need to be placed outside tend to heal nicely. Thick or raised scars or keloids are extremely rare after rhinoplasty.

The goal of rhinoplasty is to bring the nose in harmony with the rest of the face. Typically, after rhinoplasty, people will comment on the fact that you look better, but most may not notice that your nose is different. After rhinoplasty, people will tend to notice other features of your face, previously masked by your nose, such as your eyes and your lips.

One week is typically enough time for returning to work or school. At that time the dressing is removed from the nose and make-up may be worn on the face.

Not everyone will get bruising around the eyes after rhinoplasty. If black eyes do appear, they typically resolve within 7-10 days after the procedure. We will prescribe specific healing supplements to help lessen any bruising that may occur.

Typically, you can breathe through your nose after surgery. There will be some swelling of the nose during the healing process, which will make you somewhat congested, similar to having a cold.

Many patients just need work done on the cartilages of the nose and the bone may be left untouched. People with wide nasal bones, asymmetric nasal bones, or large bony humps may require reshaping of the nasal bones.

The discomfort after rhinoplasty is typically minimal. Most of our patients may need to take prescription pain medication on the first night after the procedure but by the next day they no longer need to. Many patients only need to take over the counter medication to deal with any discomfort.

Rhinoplasty can be performed on young men and woman after their facial growth has been completed. This typically translates into age 15-16 for woman and 16-17 for men.

As long as the individual is in good health, there is no upper age limit for rhinoplasty.

If there are allergies or sinus infections present, rhinoplasty surgery can still be performed. If there is an active sinus infection present, it is a good idea however to get the condition under control before proceeding with a rhinoplasty procedure. Many people suffering from allergies and sinus conditions may also have a deviated septum. Correction of the deviated septum at the same time as the rhinoplasty can often help improve their breathing passages.

Eyes / Blepharoplasty

Blepharoplasty will not correct changes in the skin quality including pigmentation, discoloration or wrinkles. The procedure may be coupled with skin resurfacing techniques including peels or laser that will improve the skin quality. The dynamic lines that are seen at the corner of the eyes often termed “crow’s feet”, are a consequence of the orbicularis oculi muscle that underlines the skin. These wrinkles are not affected by blepharoplasty and are best treated with Botox® treatments.

Occasionally patients seeking cosmetic improvement may have drooping eyelids on one or both sides (ptosis). In these instances we may recommend reattaching the upper eyelid muscle to lift the eyelids. In some people the lower eyelid may be sagging. In these cases we may recommend a tightening procedure to lift the lower eyelid (lateral canthoplasty).

Depending upon the scope of the procedure, the operation takes approximately 1 to 3 hours to complete. The procedure may be completed only on the upper eyelids, the lower eyelids or both concurrently.

Blepharoplasty or eyelid surgery is routinely performed using a local anesthetic and may include the use of sedation (oral or intravenous). In some situations a general anesthetic will be recommended. Anesthesia options will be discussed with a focus on comfort and safety.

It is important to avoid any medications that thin the blood and encourage bleeding before surgery, these include: aspirin, ibuprofen and others. We will review your medications and direct you to use appropriate supplementation before and after surgery.

It is also important that the blepharoplasty patient arrange for a caretaker for the first 24 hours after their surgery. Patients need a responsible caretaker to be with them for the first night. Patients are not allowed to drive themselves or take public transportation alone to home because of the effects of the anesthesia. Our surgery staff will ask for the name and contact information of the person picking you up and staying with you.

After your surgical procedure, when you arrive home, we encourage you to relax keeping your head elevated on several pillows, at a 30-40 degree angle. For one week you should sleep on your back with your head elevated.

For the first 48-hours, you should use ice compresses over your eyes for approximately 15 minutes of every hour. This reduces swelling and aides in your comfort.

You may shower the day after your surgery and care should be taken to avoid the direct spray of water on your face.

Avoid straining or any activity that causes a feeling of pressure in the face and eyes. You may resume strenuous activity when cleared by your doctor. You may resume gradual physical exercise starting 2 weeks after surgery.

Eyeglasses can be worn after your surgery if you need them. Contact lenses can be used after a 2-week period. Sun exposure should be avoided as much as possible.

The first two days after surgery your eyes may feel dry. It is important to moisturize them with gentle lubricating eyedrops. The doctor and  the staff will guide you with additional instructions. There is initial swelling and bruising that occurs after the procedure and takes about 1 to 2 weeks to resolve. With the use of ice packs every hour while awake for the first three days, there is a reduction in the amount of postoperative swelling. Usually the patient may return to normal non-strenuous activities 1 week after the procedure and by 2 weeks the eyelids are on average 90% improved. The best result is usually seen several weeks to months later.

The surgery generally causes mild ache or discomfort which settles over less than 48 hours, and the surgery is not regarded as painful. Usually over the counter analgesia is sufficient to reduce the discomfort

Most patients experience some bruising particularly of the lower eyelids. Postoperative swelling peaks the day after surgery and mostly resolves over the next two weeks to a month.

Depends! Most people go back to work within a week if they are okay with looking like they have had surgery because of residual swelling or bruising. Many of these visible signs of surgery can be camouflaged with regular eyeglasses. A few people wait three weeks to go back to work because they do not want any visible signs that they had surgery.

In upper eyelid surgery the scar is hidden in the upper eyelid crease. In a few cases the incision extends lateral to the eye in people who have extra skin to the side of the upper eyelid that they want removed. In lower blepharoplasty the incision is placed on the inside part of the lower eyelid (transconjunctival) in which case it is completely hidden. Occasionally an incision can be made in the skin just below the lower eyelashes (transcutaneous or subciliary). When fully healed the scars are not noticeable.

Blepharoplasty is a long lasting surgery and generally speaking has a lifelong result. All of us will have further aging with time and if you have blepharoplasty in your 30s to 40s, you will have further aging with time.

If you wear contact lenses it is recommended that when you are able to start wearing them that you minimize the time you have them (6-8 hours) in and use extra lubricating eyedrops during the first month after surgery.

Yes! Upper and lower eyelid surgery can be performed separately, at the same time and even with other procedures.

First some people only require surgery of the upper or lower eyelids. In people who are considering surgery to improve both the upper and lower eyelid areas it makes sense to have them performed at the same time. There are advantages to having the upper and lower eyelid surgeries at the same time. The recovery period for doing one is the same if you do both, so you save on the time it takes you to recover. Combining the surgeries in one setting will cost less than if they are performed separately.

Chin Enhancement

If you have a “weak chin” you are a potential candidate for a chin implant. Chin implants can benefit men and women alike; however, not all men and women are candidates for this plastic surgery procedure. For chin implant surgery to be successful, your jaw and teeth must function properly. If your chin is mildly weak and your jaw and teeth are functioning well, chin implants may be an ideal way to achieve the look you want.

The most common technique used to place the implant is performed by making a small incision under your chin. If the chin implantation procedure is performed at the same time as a face lift, your surgeon will generally use the same below-the-chin incision for both procedures. The chin implant is inserted and secured in place. This can be done by slipping the implant into a well-fitting pocket and possibly attaching it to bone or soft tissue. The chin implant procedure is finalized with sutured closure of the incision. In all, the surgery takes approximately 30 to 45 minutes. The procedure will take longer if additional procedures are being performed simultaneously.

As an alternative to chin implants, a procedure called sliding genioplasty, or chin advancement may be recommended. This procedure involves cutting the chin bone horizontally and rotating it to make your chin straighter. Sliding genioplasty can also make your chin longer or shorten it vertically.

Chin augmentation recovery time varies based on whether the implant procedure was performed alone or in conjunction with other procedures. If you had just the chin implant, you should feel fine the day of the surgery, although your surgeon will probably advise you to take it easy. You may have mild soreness in the chin area and some swelling. The chin area may feel tight due to the increased volume provided by the implant and the body’s swelling response. The pain medicines are usually needed, if at all, for only a few days. You may be advised to keep your head elevated to minimize swelling, which is temporary.

There will be no visible scarring if the chin implant is placed via an incision in your mouth. There may be a very small scar if the implant is placed via an incision under your chin which is normally only noticed by you.

As with any surgery, postoperative after effects and potential risks accompany chin implant surgery. Your lip area may feel numb, like after a dentist visit, if the incision was made through the mouth. Feeling should return the following day. Your chin area will be mildly swollen and sore. Sensation to the area may be changed. This may be a temporary or permanent change. As with any type of surgery, infection, though rare can occur and an antibiotic may be prescribed.

Lips

Ideal candidates for lip augmentation are in good overall health and are bothered by thin lips or signs of aging in their lips and the surrounding areas.

While treatment with fillers usually takes just a few minutes to perform, fat transfer, lip lift, or lip implant surgery can take upwards of an hour to complete.

Various fillers are commonly used to enhance the lips. Filler lip enhancement lasts for months. Touch ups to maintain your desired appearance may be required every 6 to 24 months. In some cases lips can be enhanced with a fat transfer. Fat transfer is long lasting, although secondary treatments may be needed because some of the fat will be absorbed by the body. Lip implants provide permanent lip augmentation, making them an alternative option for individuals with thin lips.

Compared with other cosmetic procedures, the risk of complications from lip enhancement is relatively low. However, it is a good idea to be aware of possible complications before undergoing any type of procedure. We will discuss all possible risks associated with each lip enhancement procedure with every patient so an informed decision can be made.

The length of lip augmentation recovery varies depending on the specific type of lip enhancement procedure performed. However, most patients can return to their daily routine within a day, unless a general sedative was used to complete the procedure. After any type of lip enhancement procedure swelling and bruising can be expected. These effects may last a week or so if fillers are utilized for the augmentation, and up to three weeks if lip implant placement is performed. Discomfort during the lip enhancement recovery process is minimal and rarely requires management with prescription pain medication.

Breast Augmentation

Generally, most healthy women are candidates for breast augmentation surgery. Age is not a contraindication to breast implant surgery, and it is not uncommon for women in their 50’s or later to undergo the procedure. Breast augmentation can enhance your appearance and your self-confidence. Before you decide to have surgery, think carefully about your expectations and discuss them with us. If you’re physically healthy and realistic in your expectations, you may be a good candidate.

Popular approaches are through a small incision along the edge of the lower part of the areola or an incision in the central part of the fold under the breast. A small opening is made and a pocket for the implant is developed under the breast tissue and edge of the muscle.

Both saline and silicone breast implants have their advantages and disadvantages. Most women with good breast skin quality and adequate breast tissue are good candidates for saline breast implants. Saline implants may be less expensive than silicone breast implants

Generally, silicone implants are for women with thin soft tissue coverage, poor skin quality, or for those where price is less of a factor. Silicone implants have a more natural feel to them and have less rippling or wrinkling.

We will review the details of your surgery. You will be given specific instructions in preparation for surgery and what to do after surgery. Your surgery will be performed in the Burbank Plastic Surgery Center. The center is located directly adjacent to the offices in the same building.

Recovery from surgery is pretty quick. The first night you are sore and resting. The next day you are up and about. Most patients are off narcotic pain medication after a couple of days. You can usually drive after you stop taking narcotic pain medication. Patients usually go back to “desk Job” type of activities during the first week after surgery. You usually feel that you have recovered from surgery 4 weeks after surgery. Your final cosmetic result is usually visible 2 months after surgery.

A small amount of risk is always associated with any surgical procedure. Most patients have a very low risk of complications from breast augmentation surgery. We will discuss all risks with you during your consultation and preoperative visits.

Breast Reduction

If your breasts are causing you pain and affecting your ability to enjoy life than you are a candidate for breast reduction. If you are at a stable weight or at your ideal body weight you are a candidate. If you are planning to lose weight than you are better off and will get a better result if you lose the weight before surgery.

This is the major issue with breast reduction surgery. You will have scars on your breasts. After breast reduction you will have significantly smaller breasts that are lifted and have a better shape. The scars correlate with the amount of work that needs to be performed. The length of your scars will mirror the amount of excess skin you have. The saggier and larger the breasts, the longer the scars. Two techniques are mainly used; A vertical lift results in a scar around the lifted areola and down to the breast fold under the breast. An inverted T type reduction will have the same scar but an additional scar along the breast fold under the breast. In some case a hybrid scar will be needed, having a scar only partially along the breast fold under the breast.

Predicting your breast size after surgery is difficult to do precisely. The goal of the surgery is to create beautiful breasts that are smaller, lifted and have better shape. The most common way we communicate is by using bra cup size which varies greatly depending on the manufacturer. The expected size after surgery will be discussed with you during your consultation and preoperative appointment.

If the breasts are large enough to be causing pain and affecting your function, surgery may be covered by your insurance plan. Insurance plans usually require a minimal amount of breast tissue to be removed. The amount necessary for insurance authorization is based on a formula using your height and weight (Schnur scale). Burbank Plastic Surgery will submit a request for benefits on your behalf.

The recovery is pretty quick. You are up and about the same day as surgery. Most people are back to driving in a few days, as long as you have stopped taking narcotic pain medications. Women go back to desk jobs within a week. At one month you usually feel like you have recovered and are back to doing all activities including exercise. The final cosmetic results take longer and are evident at two months.

The most discomfort is the day of surgery. The discomfort is controlled with pain medication. Icing is helpful. The following day everything is usually much better. Expect improvement everyday thereafter.

Breast Lift

The scar depends on the amount of sagging. The more sagging you have, the greater the lift needed resulting in longer scars. The smallest lift will have a scar around the areola. With greater sagging a scar may extend down to the breast fold under the breast (vertical or lollipop). If you have a lot of sagging the scar will go along the breast fold under the breast. Yes. During a breast lift the sagging skin is removed making the breasts smaller. The lifted breast will be firmer and have a much better shape. Some women will choose to have a breast implant placed at the same time as having a lift performed. Other women will get a lift and afterwards decide whether they want a breast implant placed at another setting.

Tummy Tuck / Abdominoplasty

Candidates for a tummy tuck are men or women who are in relatively good health, are minimally or moderately overweight, and bothered by a large fat deposit and loose abdominal skin that won’t respond to diet or exercise. The tummy tuck surgery is particularly helpful to women who, through multiple pregnancies, have stretched their abdominal muscles and skin beyond the point where they can return to normal. Loss of skin elasticity in older patients can be improved. Also the large volumes of excess skin produced by significant weight loss are well addressed by the tummy tuck procedure. Patients who intend to lose a lot of weight should postpone the surgery until they have reached their final weight. Also, women who plan future pregnancies should wait, as vertical muscles in the abdomen that are tightened during surgery can separate again during pregnancy.

A thigh lift procedure takes about 1-2.5 hours to complete.

After the procedure, there will be swelling and bruising at the incision sites. It is common to have some discomfort, which can be controlled with medication. You may be asked to keep a compression bandage in place to help minimize the swelling. It is important that you start walking as soon as possible.

You should be stabilized at or near your ideal weight before having an Arm lift procedure.

Arm lift scars vary depending on how much skin needs to be removed. Traditionally an arm lift scar will go on the inner aspect of the upper arm. Sometimes the scar will heal and be more visible than we like. In certain individuals the scar can be improved with a scar revision.

Most patients can return to non-strenuous work after 2 weeks. Strenuous work or exercise should not be resumed until at least 4 to 6 weeks have passed. It is important that your incisions are not subjected to excessive force, abrasion, or motion during this time.

Mommy Makeover

Mommy Makeover surgery is performed to correct the changes to the body after pregnancy. The breasts and the belly are the most affected areas after pregnancy. The breasts are lifted. If there is volume loss from pregnancy and breast feeding, this can be remedied with the addition of a breast implant.
